Important: Submission Instructions

1. All GOCCs/GFIs are required to fill-up and submit the Corporate Governance (CG) Scorecard Questionnaires (Self Assessment) to the Institute of Corporate Directors (ICD) not later than 24 July 2009.

Further, soft copies shall be saved as a PDF file but it is not encouraged to submit documents that were SCANNED and saved as PDF file.

b. For hard copy, kindly submit to the ICD at 24th/F, Yuchengco Tower, RCBC Plaza, 6819 Ayala Avenue, 1200 Makati City copy furnish the DOF-CAG at Rm 602, 6th Floor, EDPC Bldg. BSP COmplex, Roxas Boulevard, Manila.

“Note: All GOCCs/GFIs are requested to submit all the documents in soft copy via e-mail.”

Reminder:

For validation purposes, kindly specify the complete website address/links and source data including section / paragraph / page numbers, and submit the same together with all the necessary documents in pdf file to support your claim on the CG Scorecard Questionnaires.
